Job Summary: The Certified Nursing Assistant (CNA) in a Skilled Nursing Facility (SNF) provides direct patient care and supports nursing staff in delivering individualized care to residents. The role involves assessing resident conditions, documenting changes, assisting with daily living activities, and promoting effective communication among team members, residents, and families. The CNA ensures timely response to resident needs and contributes to a collaborative care environment. Responsibilities: Receive and give report on resident conditions to assure understanding of care plans.
Make rounds on each resident, assess new residents and those who are ill, injured, or have fallen.
Document assessments in accordance with facility policy.
Notify nurses of any changes in resident conditions or needs promptly.
Assist with direct patient care and ensure call lights are answered promptly.
Promote good working relationships with other departments, co-workers, residents, families, and physicians.
Perform other duties as assigned within the facility.
Required Skills & Certifications: Active California CNA license.
Good communication skills and ability to work effectively as a team player.
Basic knowledge of resident care and assessment.
Ability to document care and observations accurately.
Preferred Skills & Certifications: Experience working in a Skilled Nursing Facility or similar long-term care setting.
Familiarity with electronic medical records and documentation systems.
Special Considerations: Must be willing to work some weekends and holidays.
May be required to float to other units as necessary.
Adherence to facility policies and procedures is mandatory.
Scheduling: All shifts available, including day, evening, and night shifts.
Shift assignments may include weekends and holidays.
