FCH seeks to empower BSW customers to reach optimal health by integrating faith communities with health care to provide more effective patient navigation, education, and social support, with the goal of creating healthier communities. We operate by partnering volunteers from faith communities and universities to meet the non-medical needs of BSW discharged patients through a ministry of presence, keeping health top of mind, and social support. Essential Functions of the FCH Manager role:Oversees the FCH program and ensures outcomes; innovatively shifts the strategy for greatest impact and long-range program sustainability.Conducts quality improvement, fundraising, and grant-writing.Oversees system staff, creates and maintains a positive and collaborative culture; collaborates with system community outreach efforts, and engages with multiple communication venues to advance the visibility of FCH. Establishes and maintains relationships with faith leaders to leverage faith communities to help improve health outcomes in the community.Establishes and maintains relationships with university leaders to leverage university volunteerism to improve health outcomes in the community.Explores and integrates the ways faith can positively influence health outcomes and community well‑being.Explores how spiritual practices intersect with public and community health and integrates learnings into the strategy and operations.
