As a Field RN Home Care, you will support clients, caregivers, and families by ensuring quality care, proper documentation, and a positive home care experience. This is an ideal role for an RN who enjoys working independently, building relationships in the community, and supporting both clients and caregivers in the field.

Responsibilities:

• Travel to client homes across assigned counties
• Complete nursing assessments and reassessments as needed
• Develop, review, and update client care plans
• Monitor client condition and report changes as appropriate
• Provide support, guidance, and supervision to caregivers
• Ensure care is being provided according to the client's needs and care plan
• Communicate with clients, families, caregivers, and office staff
• Maintain accurate and timely documentation
• Help ensure compliance with company policies and home care requirements
• Promote a compassionate, respectful, and professional care environment

Requirements:

• Active RN license
• Prior home care experience preferred
• Willingness to travel to client homes in multiple counties
• Valid driver's license and reliable transportation
• Strong assessment, communication, and documentation skills
• Ability to work independently and manage time effectively
• Professional, compassionate, and reliable
• Must meet all company hiring and onboarding requirements
