Our community is uniquely diverse, and it’s our aim to fulfill the unique needs of our patients through excellent individualized care.Our focus is to help facilitate a smooth and safe transition home and provide life-changing service with the amount of care and dignity our patients and their families deserve.Sequoia Home Health is growing and looking for experienced, compassionate Physical Therapist Assistant (PTA) to join our team!JOB SUMMARYThe Physical Therapist Assistant (PTA) provides physical therapy services under the direction of the physical therapist and according to the physician's plan of care.DUTIES & RESPONSIBILITIESProvide direct physical therapy according to directions of physical therapist and in accordance with the physician's plan of careTreat patient to relieve pain, develop or restore function, and maintain maximum performanceDirect and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic trainingUtilize physical agents, including, but not be limited to heat, cold, water, light, electricity, ultrasound and massageObserve and report to the physical therapist and the Organization personnel the patient’s reaction to treatment and any changes in patient’s conditionInstruct patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devicesMaintain necessary records; clinical notes and conference notes, which will be incorporated into the patient’s clinical record in prescribed time framesParticipate in Organization Home Care Team Members meetings and Organization in-service Education ProgramJOB REQUIREMENTSActive CA PTA LicenseCurrent CPR Certification1+ Year of Clinical ExperienceMust be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working.Why Sequoia?Sequoia Home Health and Hospice is part of the Cornerstone Group with close to 80 agencies throughout the country.
