Home Health Registered Nurse (RN)

We are seeking a dedicated Home Health Registered Nurse (RN) with adult home health experience to provide exceptional care to patients in their homes. The ideal candidate will be proficient in handling PICC lines, ports, trach care, Pleurx, and patient education, adhering to Medicare guidelines related to home care and completing OASIS documentation accurately.

Responsibilities

• Conduct patient assessments and manage care plans.
• Administer medications and provide treatments as prescribed.
• Perform OASIS documentation and ensure compliance with Medicare guidelines.
• Educate patients and their families on health management and care expectations.
• Manage and care for central lines, tracheostomies, and other medical devices.

Required Experience / Certifications / Licensure

• Must have an active RN license in Michigan.
• 1 year of adult home health nursing experience required.
• Proficient in using Epic as a charting system.
• Basic Life Support (BLS) certification is required.
• Experience with Medicare and OASIS documentation.

Schedule

This position requires working from 08:00 AM to 04:30 PM, with every 5th weekend required.
