Thank you for your interest in volunteering for the Brentwood Union School District! Those seeking to be a Classroom Volunteer/Volunteer Coach/Chaperone will need to complete and submit the following: Complete this Volunteer Application (attach identification, TB test or TB Risk Assessment form and mandated reporter training)

Once we receive your application, you will receive an email from Keenan, our compliance platform, with a link to complete the required Mandated Reporter Training. All above completed documents can be attached to your application in Frontline.

Important Notice

New TB Testing Information

All newly hired employees, current employees, coaches or volunteers due for a four (4) year TB screening are required to provide the District with proof of freedom from TB.

In addition to the current TB skin test (PPD) or X-ray, there is a new option available. It is the TB Risk Assessment Questionnaire and Certificate of Completion. This must be completed by a licensed health care provider (physician, physician assistant, nurse practitioner or registered nurse). This form may be provided by the licensed health care provider or is also available at the following link: qov/proq rams/tb/Docu ments/TBC B-Adu It-TB-Risk-Assessment-for-schools. pdf Any cost for the completion of this risk assessment must be paid by the employee/volunteer. If no risk factors are identified on the TB Risk Assessment, no TB test is required.

If there are risk factors identified on the TB Risk Assessment, the employee/volunteer must provide proof of freedom from infectious tuberculosis.

We continue to partner with Kaiser Permanente's Occupational Health Department. Or, as always, you may see your own personal physician. Kaiser members may drop in at the Injection Clinic for no cost.

There is a contracted BUSD flat fee: employee and/or substitute/volunteer MUST BRING THIS DOCUMENT as proof of Brentwood USO applicant/employee status. Please call Concentra to confirm the contracted rate.

They do not accept other insurance; drop-ins accepted & no appointments needed.
