To support a growing Chronic Care Management team, the full-time California Licensed Chronic Care LVN will conduct high-volume outbound patient calls, perform assessments, and assist with care plans in a remote clinical environment. Key Responsibilities Manage inbound and outbound telephonic patient interactions, maintaining a call volume of 70 to 90 calls per day Conduct and document monthly Chronic Care Management care plan calls, ensuring timely outreach and comprehensive care coordination Collaborate with providers to coordinate effective patient care through telephonic communication and accurate real-time documentation Required Qualifications Active LPN/LVN Compact License required Active, unrestricted LVN license issued by the California BVNPT Passionate about patient care with the ability to relate with empathy and compassion Proficient with computers, EMRs, and telehealth tools Strong communication and organizational skills
