Location & Compensation Hiring in Oak Cliff, TX Per Visit Rate Job Summary We are seeking a skilled Home Health Physical Therapist (PT) - PRN - Per Visit to join our team in Dallas, TX. The ideal candidate will have experience providing PT services in patients' homes. As a Home Health PT, you will assess patients' needs, develop treatment plans, and provide therapy services to help patients regain mobility and independence. Responsibilities Conduct initial assessments of patients' physical abilities and develop treatment plans Provide therapy services to patients in their homes Monitor patients' progress and adjust treatment plans as needed Communicate with patients' physicians and other healthcare professionals to coordinate care Document patients' progress and maintain accurate records Provide education to patients and their families on how to manage their conditions and prevent further injury Requirements Current license to practice as a Physical Therapist in Texas Minimum of 1 year of experience in home health Excellent communication and interpersonal skillsAbility to work independently and as part of a team Strong problem‑solving and critical thinking skills Valid driver's license and reliable transportation #J-18808-Ljbffr
