Description as a Direct Support Professional:
ASG Home Care NJ LLC is seeking a dependable Direct Support Professional (DSP) to support a 66-year-old female individual with transportation and community-based support needs. The individual lives in Keyport and attends a day program in Dayton (Dawn 2 Dusk). This role includes morning transport to program and afternoon pick-up with additional community support before returning home.

Schedule & Hours:
• Monday–Friday
• Morning: approx. 1 hour (home → day program; program starts at 8:30 AM)
• Afternoon: approx. 2–3 hours (2:30 PM pick-up, community activity, return home)
• Estimated total: approx. 3–4 hours/day, Monday–Friday (about 15–20 hours/week)

Location:
• Home: Keyport, NJ
• Day Program: Dayton, NJ
• Approx. 20 miles each way

Responsibilities as a Direct Support Professional:
• Provide safe, reliable transportation to and from program
• Support participation in simple community activities after program
• Provide supervision and support during transport and scheduled time
• Maintain professional communication with family and agency staff
• Complete required documentation and EVV requirements

About the Individual:
• 66-year-old female (Tier D)
• Lives with aging parents
• Very sweet; does best with staff who are patient, kind, and gentle

Requirements as a Direct Support Professional:
• Valid driver’s license and reliable vehicle
• Clean driving record
• DSP experience preferred (IDD experience strongly preferred)
• Must meet all NJ DDD onboarding requirements (fingerprinting, trainings, etc.)
