About This Role

As an RN in Hospice at Cleveland Clinic, you will provide compassionate care to patients and their families during end-of-life transitions. This role is ideal for those who value autonomy and wish to maintain a healthy work-life balance while delivering exceptional care.

What You'll Do

• Provide supervision, assessment, planning, and direct patient care to hospice patients.
• Perform accurate and complete admissions assessments and re‑assessments.
• Assess patients for safety risks, abuse, and neglect.
• Assess, document, and manage patient’s pain.
• Prioritize and document patient problems.
• Administer medications and treatments safely and accurately with a focus on patient comfort.
• Coordinate care for managing a caseload of patients.
• Document changes in patient condition and effectiveness and timeliness of care/interventions.
• Evaluate, update, and revise plans of care.
• Assess educational needs of patients, caregivers, and family while considering cultural issues and other special challenges.

Requirements

Education

• Graduate from an accredited school of Nursing
• BSN preferred

Experience

• 1+ years of current clinical experience as an RN
• 1+ year of experience in an acute medical‑surgical or critical care setting

Licenses & Certifications

• Current RN License in Ohio
• Basic Life Support (BLS) Certification from the American Heart Association (AHA)
• Valid Driver’s License (VDL) and proof of automobile insurance with 100,000/300,000 coverage required
• Hospice and Palliative Care Nurse Certification preferred
