Job Details: Schedule: Monday-Friday, 8:00 AM - 4:30 PM Duration: 14-week contract Mileage Reimbursement: Yes, between patient visits Location: Renton, WA Job Type: Contract Pay Range: $53-$56/hour Pay offered is based on experience, expertise, credentialing, and education.

Duties: Provide skilled nursing care to patients in their homes as prescribed by the physician. Develop, implement, and revise individualized care plans in collaboration with physicians and other healthcare professionals. Complete comprehensive patient assessments, including OASIS assessments and medication reconciliation. Coordinate care and communicate effectively with physicians, patients, families, and interdisciplinary team members. Supervise and provide direction to Home Health Aides and LPN/LVNs in accordance with state and federal regulations. Maintain accurate, timely, and thorough documentation of all patient visits and care provided. Participate in care coordination, case conferences, quality improvement initiatives, and patient outcome management. Assist with orientation and training of new staff and serve as a preceptor when needed. Participate in weekend rotations and provide flexibility to meet patient care needs.

Requirements: Active RN License in the state of practice Current CPR Certification Current Driver's License, vehicle insurance, and reliable transportation Ability to complete OASIS assessments and manage patient care independently

Preferred Qualifications: Home Health experience preferred Experience working in a point-of-care setting preferred
