Rehabilitation Liaison

OhioHealth is seeking a Clinical Rehab Liaison to coordinate rehabilitation services and transitions of care across our healthcare network. In this role, you will complete clinical assessments, collaborate with an interdisciplinary team, and guide patients and families through the rehab continuum. You will support referrals, admissions, and discharge planning while ensuring quality, safety, and regulatory compliance.
