Your Responsibilities

• Carrying out thorough patient evaluations, ordering and interpreting diagnostic exams, and crafting customized care strategies
• Diagnosing and managing acute or chronic medical conditions, collaborating with physicians or independently per credentialing
• Providing educational support to patients and families around health maintenance, preventive measures, and chronic illness management
• Prescribing necessary medications and therapies consistent with your practice scope
• Engaging in follow-up care and chronic disease management with diligence
• Coordinating with multidisciplinary teams to deliver comprehensive, patient-centric care
• Ensuring meticulous documentation within electronic health records (EHR)

Qualifications Necessary

• A Master's degree in Nursing (MSN) or a Doctor of Nursing Practice (DNP)
• Successful completion of an accredited Nurse Practitioner program

Licenses and Certifications

• Active Nurse Practitioner license in Pennsylvania
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or equivalent specialty
• Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or capability to obtain within 90 days)
