Hospice Admissions Registered Nurse

The Admissions RN is responsible for coordinating and processing hospice referrals to ensure timely and seamless patient admissions. This role serves as a key clinical resource for referral intake, reviewing and accepting referrals from various sources via phone, fax, and electronic systems. The Admissions RN collaborates closely with referral sources, physicians, intake staff, and the clinical team to coordinate initial hospice assessments and ensure compliance with hospice regulations and organizational standards. Key Responsibilities. Receive, review, and prioritize hospice referrals to support timely admissions and continuity of care. Respond promptly and professionally to referral inquiries and intake calls. Enter and maintain accurate patient demographic, clinical, and insurance information within the EMR - Obtain physician orders, document Certification of Terminal Illness (CTI) information, and ensure referral documentation is complete and compliant.
