Responsibilities:

• Provide personalized support and assistance to individuals with disabilities in daily living activities.
• Develop and implement individualized care plans to promote independence and well-being.
• Facilitate social interactions and community engagement opportunities.
• Monitor client progress and report any changes in behavior or health to the supervisor.
• Assist with personal care tasks, including hygiene, meal preparation, and medication management.
• Maintain accurate documentation of services provided and client progress.
• Collaborate with families and other professionals to ensure comprehensive care.
• Participate in ongoing training and professional development opportunities.

Requirements:

• High school diploma or equivalent; relevant certifications preferred.
• Experience in direct support or caregiving roles is highly desirable.
• Strong communication skills and a compassionate demeanor.
• Ability to work independently and as part of a team.
• Valid driver's license and reliable transportation.
• Background check and drug screening required.
• Flexibility to work various shifts, including evenings and weekends.
