Oak Hill TGH - Program Coordinator

Overview Responsible for the delivery and implementation of behavioral health or recovery programs within a facility. Responsibilities

ESSENTIAL FUNCTIONS:

Provide patient care to include coordinating activities, providing companionship and assisting with basic daily needs (dressing, bathing, eating). Collaborate with treatment team members to assist with patient treatment and/or care goals. May serve as a co‑lead for groups. Monitor the daily well‑being of patients and identify need for social intervention. Ensure all documentation regarding the program, such as care, treatment or incidents, is completed in a clear, concise, professional and timely manner. Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

Bachelor's degree in Human Services, Social Work or related field required. One or more years previous experience in behavioral health with population of the facility preferred.

LICENSES/DESIGNATIONS/CERTIFICATIONS:

Certification in specialty of program preferred. CPR and de‑escalation/restraint certification required (training available upon hire and offered by facility). First aid may be required based on state or facility.
