Hospital Liaison Nurse LPN

Job Description The Hospital Liaison Nurse (LPN) coordinates and facilitates the delivery of home‑care services in conjunction with the home‑care team, hospital discharge planning staff and other referral sources. The LPN facilitates referrals, coordinates skilled and non‑skilled services, and other community resources through collection and input of data, interview and team collaboration in a variety of settings. Under the direction of a registered professional nurse, the LPN performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care. Location Home Care‑VNA Erie County, Cheektowaga, NY Schedule and Hours Part‑time. 8 a.m. – 4:30 p.m., 39.75 hours bi‑weekly. Weekend/holiday requirement: Yes. On‑call required: No. Work arrangement: Onsite. Qualifications High School Diploma or GED required. Associate of Applied Science (LPN) preferred. NYS Driver’s License required. Licensed Practical Nurse (LPN) required. 1 year of experience in medical/surgical nursing required; 1 year in home‑care setting preferred. Physical Requirements Weight requirement – light (up to 20 lbs) Salary Salary Range: $26.76 – $35.10 per hour.
