Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting to restore or improve physical function and mobility
• Develop and implement individualized physical therapy treatment plans based on patient assessments and goals
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory requirements
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
• Educate patients and family members on therapeutic exercises and home safety strategies

Required Qualifications

• Active Physical Therapist (PT) license in the state of South Carolina or compact eligibility
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• Experience working in a skilled nursing facility (SNF) or post-acute care environment

Preferred Experience / Skills

• Prior travel or contract therapy experience
• Familiarity with MDS documentation and Medicare guidelines in a SNF setting
