Social Services Assistant Job Summary: The Dubois Continuum of Care Community is seeking a highly motivated and compassionate

Social Services Assistant

position. Responsibilities: Assist the Social Services Team in providing emotional support and guidance to residents and their families

Coordinate and facilitate social activities, events, and programs designed to promote resident engagement and socialization

Conduct assessments and develop care plans tailored to each resident's unique needs and preferences

Communicate effectively with residents, families, and interdisciplinary team members to ensure seamless care coordination

Maintain accurate and detailed records of resident interactions, assessments, and care plans

Participate in ongoing training and professional development to stay current with best practices and regulations
