We are seeking a full-time Physical Therapist with a focus on acute care and inpatient rehabilitation. The ideal candidate will play a crucial role in assessing, planning, organizing, and implementing rehabilitation programs to improve patient mobility, relieve pain, increase strength, and correct disabling conditions resulting from diseases or injuries.

The Physical Therapist will be responsible for the following:

• Developing and implementing individualized physical treatment programs to maintain, improve, or restore physical function.
• Completing and documenting patient evaluations and tests in accordance with established standards and regulatory requirements.
• Updating and managing the patient's Plan of Care based on assessment data and diagnosis.
• Providing and progressing treatment for patients according to their Plan of Care.
• Ensuring all documentation related to charge capture is completed accurately.
• Coordinating patient care efficiently with other healthcare providers and support team members.
• Working flexible shifts, including varying days and hours as needed.
• Supervising assistants and aides in clinical settings.
• Serving as a clinical instructor for assigned students.

Preparing and completing accurate documentation related to patient therapy activities in medical and departmental records.

Performing general clerical tasks, including answering phones and updating patient appointments.

Managing and maintaining patient electronic health records accurately.

• Participating in quality improvement processes and interdisciplinary projects.
• Maintaining a clean and safe working environment, including the cleaning and disinfecting of equipment.
• Using equipment according to manufacturer guidelines and reporting any malfunctions.
• Ensuring patient privacy by maintaining confidentiality of all patient Protected Health Information (PHI).
• Adhering to all state and federal regulations regarding service administration and documentation.
• Staying compliant with departmental and organizational requirements and responsibilities.
• Retaining certification and completing continuing education as mandated to meet accreditation and licensure requirements.

The candidate must be a graduate of an accredited physical therapy graduate program.

This position typically follows a day shift of 5 days per week, operating from 8:00 AM to 4:30 PM.
