Nurse Practitioner (Home Based Primary Care)- Tilton/Concord

Nurse Practitioner Position

This Nurse Practitioner position is located in the Home Based Primary Care service line at the Tilton/Concord NH location. This position is full-time at 40 hours per week.

Included but not limited to:

• Provide a full range of services emphasizing quality, timeliness, and the advancement of the VA mission.
• Provides advanced clinical practice, consultation, and management.
• Practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The NP has prescriptive authority for a panel of patients or program as indicated by their position.
• Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
• Develops and implements an individualized plan to achieve optimal population health and disease management.
• Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members.
• Orders and interprets diagnostic studies.
• Prescribes pharmacologic and non-pharmacologic interventions.
• Initiates referrals/consults to specialty services.
• Provides education to patients and their families.
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

• Other duties as assigned.

Work Schedule: Full-Time, Monday- Friday 8:00 A.M- 4:30 P.M.

• Current and former Federal employees must submit copies of their most recent SF-50, (Notice of Personnel Action). Pay: Competitive salary, regular salary increases, potential for performance awards
