TeleMate Health is looking for a compassionate, dependable Licensed Practical Nurse (LPN) to join our team as a Clinical Liaison . This is a patient-facing role focused on helping patients transition from healthcare facilities and hospitals back to their homes with the support and services they need to remain connected to their healthcare team.

Position Overview

The Clinical Liaison serves as an important connection between TeleMate Health, healthcare facilities, hospital teams, patients, and their caregivers .

The primary responsibility is to meet patients at the bedside, educate them about TeleMate Health, and help coordinate their transition home with TeleMate services. The Clinical Liaison will work closely with facility and hospital staff to identify appropriate patients, complete the enrollment process, and ensure the patient understands what to expect once they return home.

When bedside liaison responsibilities are lighter, this position will also provide clinical support to TeleMate Health's health monitoring team.

Primary Responsibilities

• Visit patients at the bedside in skilled nursing facilities, senior living facilities, and hospitals .
• Meet with patients and caregivers to explain TeleMate Health and how our services can support them after discharge.
• Educate patients on remote patient monitoring and other available TeleMate Health services.
• Complete patient enrollment, consent, demographic, and clinical information as required.
• Coordinate with facility staff, hospital staff, case managers, discharge planners, and TeleMate Health team members to support a smooth transition home.
• Help ensure patients understand their equipment, services, and next steps before leaving the facility.
• Communicate patient needs, concerns, and barriers to the appropriate TeleMate Health team members.
• Assist with identifying patients who may benefit from TeleMate Health services.
• Provide clinical support to the TeleMate health monitoring team when not working in the field.
• Participate in patient outreach and follow-up as appropriate and within the LPN's scope of practice.
• Document patient interactions and activities accurately in the TeleMate Health system.
• Maintain professional relationships with facility and hospital partners.
• Represent TeleMate Health professionally and provide a positive patient experience.

What We're Looking For

• Current, unrestricted Tennessee LPN license .
• Previous clinical experience; experience in SNF, hospital, home health, case management, or care coordination is preferred.
• Strong clinical knowledge and the ability to apply nursing judgment in a variety of patient situations.
• Strong critical-thinking and assessment skills , with the ability to recognize changes in a patient's condition and appropriately communicate concerns to the nursing or provider team.
• Ability to review patient information, identify potential concerns, and determine when additional clinical follow-up or escalation is needed.
• Comfortable providing clinical support to the health monitoring nursing team when not working in the field.
• Strong communication and interpersonal skills.
• Comfortable speaking with patients, families, nurses, providers, and facility staff.
• Ability to work independently while remaining connected with a clinical team.
• Organized and able to manage multiple patients and responsibilities.
• Comfortable using an electronic medical record and other computer-based systems.
• Reliable transportation and willingness to travel throughout the Memphis area and surrounding communities.
• When not working in the field, the ability to work effectively and professionally from a home office with reliable high-speed internet and an appropriate, private workspace.
• Compassionate, professional, and patient-focused approach.
• Ability to recognize patient concerns and appropriately communicate them to the clinical team.
• Love for geriatric and senior adults

Selection Process

Because this position combines clinical nursing, patient interaction, and facility relationship management, candidates will meet with members of both the TeleMate Health Nursing/Clinical team and Business Development team during the interview process.
