Physical TherapistPride Health is hiring a Physical Therapist to support our client's home health program in Nutley, NJ (Coverage within Newark, Nutley, Belleville, and Bloomfield in New Jersey). This is a 6-month assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!Job Name: Physical TherapistLocation: Nutley, NJ 07110 with Travel within Newark, Nutley, Belleville, and Bloomfield in New JerseyJob Type: Local ContractDuration: 6+ months Contract (Possibilities of extension)Pay rate: $50 - 56/hr. On W2 (Negotiable - Pay Rate is based on experience)Schedule: Mon – Fri (5 days/ wk.) from 8 AM to 4:30 PM (30-hour weekly guarantee)Mileage: Reimbursement provided (from second patient to last patient)Job Responsibilities:Perform Start of Care (SOC) visits, completing initial assessments and establishing individualized Plans of Care (POC) in accordance with physician orders.Conduct thorough physical therapy evaluations assessing strength, mobility, balance, pain, and functional limitations.Provide skilled therapy interventions during routine and follow-up (revisit) appointments.Perform re-evaluations to assess patient progress, update treatment plans, and adjust goals as needed.Develop and implement evidence-based treatment plans tailored to each patient's needs and goals.Educate patients and caregivers on home exercise programs, safety precautions, and disease management.Complete timely and accurate clinical documentation in compliance with regulatory and organizational standards.Coordinate care with physicians, interdisciplinary team members, and caregivers to ensure optimal patient outcomes.Conduct discharge visits, including final assessments, patient education, and transition planning.Qualifications:Graduate of an accredited Physical Therapy program (bachelor's or higher).Active, unrestricted Physical Therapist (PT) license in New Jersey.Minimum 1 year of clinical experience (home health experience preferred).Valid driver's license and reliable transportation.Proficiency with Homecare Homebase/ PointCare (preferred).Strong working knowledge and experience with OASIS documentation.Pride-Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.Equal Employment Opportunity Pride-Health is an equal opportunity employer.
