Intake Specialist - Home Care (Bilingual English/Spanish)

People Care,Inc is looking for an experienced   Intake Specialist   with a background in   home care intake   to manage patient referrals and ensure a smooth and timely start of care within a fast-paced home care setting. This role is critical in ensuring accurate intake, strong communication with referral sources, and high conversion of new cases.

What You’ll Do

• Manage the full intake process for new home care patients
  
• Receive and process referrals from hospitals, physicians, and community sources
  
• Verify insurance eligibility (Medicaid, Medicare, managed care) using ePaces
  
• Collect and review patient demographics and required documentation
  
• Conduct intake calls with patients and families
  
• Schedule initial assessments and coordinate start of care
  
• Follow up on pending referrals and missing documentation
  
• Track intake performance (referrals, conversions, SOCs)
  
• Maintain accurate data in HHA Exchange and internal systems
  
• Handle high call volume in a fast-paced environment while maintaining professionalism

Requirements

• 1+  years of   direct HOME CARE intake experience (required)
  
• Bilingual English/Spanish (required)
  
• Strong knowledge of HHA Exchange and ePaces
  
• Familiarity with Medicaid, Medicare, and managed care plans
  
• Excellent communication and phone skills
  
• Strong attention to detail and documentation accuracy
  
• Ability to multitask and manage high-volume workflows
