Field-Based Hospice Community Liaison - Impact & Growth
Three Oaks Hospice, Inc.
Three Oaks Hospice, Inc. is seeking a Community Liaison to promote hospice services, build referral networks with physicians, hospitals, discharge planners, and other programs within the assigned territory. You will identify admissions and enroll patients into hospice in a timely manner. This field-based role requires travel across the territory, strong communication skills, and the ability to collaborate with healthcare professionals to grow referrals and raise community understanding of #J-18808-Ljbffr Three Oaks Hospice, Inc.
