*About This Role*

Mannacare Health and Wellness Medical Center is seeking a licensed Nurse Practitioner to join our primary care team in Lima, Ohio. This role provides comprehensive primary care services to patients across the lifespan, including routine exams, chronic disease management, preventive care, and diagnosis/treatment of acute conditions, working closely with our clinical and administrative teams to deliver consistent, high-quality patient care.

*Requirements*

* Active Ohio APRN license with Family Nurse Practitioner (FNP) certification
* Active Ohio APRN license with Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) certification
* Active Ohio APRN license with Adult Nurse Practitioner (ANP) certification
* Active Ohio Physician Assistant (PA-C) license with primary care experience
* Active DEA registration or ability to obtain one
* Experience with Tebra or comparable EHR/practice management software preferred
* Experience in a primary care or family medicine setting preferred
* Strong communication skills and comfort working within a collaborative, multi-provider practice
