RN Case Manager
Midwest Health
Community Health Network CHN is seeking a Full or Part Time RN case manager for Wayne County. Case Managers are responsible for completing OASIS assessments at start of care, recertification and completion of supervisory visits. Skilled visits as caseload requires with reasonable productivity requirements. Manages LPN or RN that assist with skilled visits that CM manages. On call rotation, with compensation allotted for on call week. Would be responsible for some skilled visits and medication passes. What we offer:
