Job Title: Case Manager RN Profession: RN Specialty: Case Management Duration: 13 weeks Shift: Day Shift Hours per Shift: 8.5 hours Experience: Minimum 2 years License: RN License Required Certifications: None specified Description: Alternating weekends are required. Position is located in a facility with 350 beds. The case management team consists of various staff members, including case managers and social workers. Patient ratio on the unit is approximately 25 patients per case manager. Candidates must have case management experience in an acute care setting. Experience with electronic medical records systems is preferred. This position requires flexibility to work every other weekend and holidays. Candidates without the specified skills will not be considered. The unit incorporates modern equipment and technology to support patient care.
