• Mon - Fri, 8 am - 5 pm -- rotating weekends required
• Guaranteed 40 hours per week
• 6.5 visits per day
• Home health physical therapy setting
• EMR: Home Care Home Base (HCHB)
• SOC, recertifications, ROC, evaluations, and daily visits
• Common diagnoses -- CHF, orthopedic conditions, pneumonia
• 13-week assignment
