Registered Nurse for Hospice Unit

Duties

The VA New Jersey Healthcare System is currently recruiting for one Registered Nurse to serve on our Hospice unit within our Community Living Center. Duties for this position include, but are not limited to:

• Provides competent, evidence-based care to assigned residents and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting.
• Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
• Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
• Administers medications and procedures per established policies and guidelines.
• Influences care outcomes by collaborating with members of the interdisciplinary team.
• Participates in the unit/program level quality improvement process and initiatives as well as customer service programs.
• Collaborates with the Minimum Data Set (MDS) coordinator to ensure documentation is accurate and reflective of the Resource Utilization Group (RUG) score.
