Essential Duties and Responsibilities: 

The Adult Skills Center (TASC) provides innovative and proactive services that empower individuals with diverse intellectual, developmental, and mental health needs to achieve their highest level of Independence and realize their greatest potential. 

The Supported Employment Case Manager will specialize in supporting clients throughout their supported employment programs including Paid Internship and Community Integrated Employment through Regional Center, DOR, and/or SSA. Liaison between TASC, client, the IPP team, and job sites following best practices and point of contact standards. Provides support to the client, staff, and job site as needed. Creates contacts with potential employers in the community based on the needs of clients assigned. Spearheads the Discovery process, case management, file creation, identifying client needs and skills, Acts a resource guide for clients and staff, tracks documentation of services and all interactions. Participates in case review meetings with counselors / referring agencies for updates and communicating challenges and outcomes to the team. Produces and submits documents pertaining to client referrals and progress.

Primary Duties:

Case Management

1. Maintain a caseload of program participants and support those clients as they seek to gain and maintain employment.
2. Works directly with Department of Rehabilitation, Regional Center, Social Security and other agencies to place clients in supported employment programs and manage their cases.
3. Attends all client meetings and prepares progress reports.
4. Identifies and utilizes community resources to meet client needs
5. Facilitates classes/groups on budgeting, benefits resources, etc.
6. Participates and attends on/off site staff and community meetings
7. Relies on experience and judgment to plan and accomplish goals
8. Works under general supervision or independently
9. Process DOR, Regional Center and/or SSA referrals, documents, and reports.
10. Participate in overseeing and training job coaches for specific client needs and employment settings.
11. Meet with participants, both one-on-one and in group settings, to assess strengths and barriers to employment, identify job leads, produce and refine resumes/cover letters and sharpen interview skills, among other tasks.
12. Work with a diverse set of job seekers, including but not limited to individuals who: have intellectual and developmental disabilities, have significant criminal histories, carry potential mental health, behavioral, or substance abuse concerns, have gaps in work history, and/or demonstrate interpersonal difficulties and other obstacles that impede goal attainment.
13. Develops employment opportunities in the community and maintains those relationships.
14. Act as a resource for all program participants and refer them to the appropriate service provider when the program does not fill their needs.
15. Perform client assessments and record those assessments.

Program Management

1. Participate in the hiring, training and on-going supervision of all program staff.
2. Provide supervision and mentorship to program staff including delegating duties and responsibilities according to their job descriptions, meeting together with supervisors on a regular basis, keeping written documentation of all meetings, directives, trainings, disciplinary actions, and agreements; conducting annual job performance evaluations.
3. Prepare for and participate in audits by regulatory and funding agencies.
4. Provide oversight to assigned program office space and equipment.
5. Ensure quality of services including relationships with clients, families, funding agencies, regulatory agencies, and the community.
6. Collaborate with all TASC departments and foster team engagement.
7. Participate in and lead program evolution and growth in areas of need.

TASC Leadership

1. Represent TASC within the community
2. Participate on community boards and/or committees in order to enhance community knowledge of TASC as well as provide TASC with relationships with additional community resources
3. Speak to community groups when requested by Director
4. Participate in TASC strategic plan implementation
5. Participate in committees and contribute ideas
6. Serve as a positive role model for peers and staff
7. Foster a positive work environment through constructive feedback and problem solving
8. Encourage learning, personal growth, and positive attitude
9. Foster team engagement and growth
10. Other duties as assigned

Qualifications: 

• 2 years of experience working with adults with developmental disabilities in a vocational setting required. Bachelor's degree or equivalent experience in social work or related fields preferred
• Desire to compassionately align with the client, while supporting a culture of creating self-reliance and sustainability
• Ability to establish and develop effective relationships with staff, employers, volunteers, community groups, current and potential donors, corporate sponsors, agency partners and other key stakeholders
• Strong computer skills, including demonstrated success with Microsoft Office Suite; strong skills and comfort with internet applications 
• Ability to work in multiple locations (office/remote/community)
• Must be able to successfully complete First Aid/CPR, ACRE, Core Competency Training, Direct Support Professional Training, and other trainings as required
• Must have current California driver’s license and clean DMV 
• Must have reliable transportation and current auto insurance 
• Must receive criminal record clearance through the DOJ

Physical Requirements:

2. Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards. 
3. Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards
4. While performing the duties of this job, the employee is frequently required to stand, walk, and sit. The employee must occasionally lift and/or move up to 50 pounds. 

The employee must provide physical assistance and/or push clients up to 250lbs in a wheelchair. Must be able to assist adult clients transitioning in and out of vehicle and in the restroom.  

Completion of Professional Crisis Management Certification within 90 days of hire (provided by TASC) is required for this position and is not recommended for:

1) Individuals who have mobility and/or range of motion problems 

2) Individuals with recent fractures or surgeries 

3) Individuals who have suffered any kind of joint injury requiring medical attention in the past 6 months 

4) Individuals with poor balance, flexibility or agility
