In-Home Senior Case Manager: Elder Care & Community Impact
Catholic Charities Brooklyn and Queens, Inc.
Catholic Charities Brooklyn and Queens, Inc. is looking for a Case Manager III to provide comprehensive services to homebound elderly clients. This is a full-time position focused on maintaining clients in their homes through detailed assessments and personalized care plans. The ideal candidate will possess a Bachelor’s Degree in Human services and knowledge of older adult entitlements. This role includes a variety of responsibilities such as crisis intervention, case documentation, and regular client interactions.
