Founded in 2002, AFC provides housing, healthcare, and educational support to thousands of youth each year. AFC creates an affirming, supportive environment where LGBTQ+ youth can thrive. POSITION SUMMARY The Case Management Program Manager will oversee a team of (4) Case Managers and (2) Housing Navigators to ensure the effective delivery of services to our clients (ages 16-24) at Ali's Place. This role requires strong leadership skills, extensive knowledge of social services practices, collaboration across all departments, and a commitment to serving queer homeless youth. WORK SCHEDULE: Tuesday-Saturday 12pm-8pm. Some holidays are required. WORK FLEXIBILITY: 1 remote day per week (Tuesday) KEY RESPONSIBILITIES Supervision and Leadership Provide direct supervision, guidance, and training to a team of Case Managers and Housing Navigators, ensuring delivery of high-quality, impactful services to AFC clients that utilize Ali's Place drop-in center. Ensure that DYCD and DOH outcomes are met. Collaborate with ICM Coordinator to assign new clients. Provide site supervision at Drop-In Center, by managing scheduling and coverage issues, crisis de-escalation, and other relevant needs. Conduct performance evaluations, provide constructive feedback, and lead weekly supervision to all team members. Coordinate team meetings, training, and professional development opportunities to enhance staff skills and knowledge. Foster a supportive and collaborative work environment that promotes staff growth and development. Oversee the development and implementation of individualized service plans for each client to ensure they are receiving comprehensive support tailored to their needs. Other duties as assigned. Program Development and Evaluation Participate in the development and enhancement of program initiatives, policies, and procedures to improve service delivery and meet the evolving needs of clients. Conduct regular program evaluations, collect data, and analyze trends to inform decision-making and program improvements. Quality Assurance and Compliance Ensure the timely and accurate completion of required written documentation by Case Managers and Housing Navigators. Input relevant data into designated databases (PTS, AWARDS, CAPS) to track client progress and outcomes effectively. Collaborate with the Director of Case Management to develop and implement systems for quality assurance to ensure adherence to agency policies, program standards, and regulatory requirements. Prepare for audits and collaborate with internal and external stakeholders to maintain compliance with funders expectations (e.g., DYCD, DOH). Assist in the preparation of reports, grant applications, and other documentation as needed. Monitor client progress, track outcomes, and ensure documentation is accurate and up-to-date. Collaboration Across Teams Work closely with Ali's Place leadership to coordinate services and address the needs of youth. Collaborate with a multidisciplinary care team (mental health providers, health services, residential treatment model, etc) to ensure communication and quality care provision. Participate in regular team meetings to facilitate communication and ensure collaboration. Conduct intake assessments and crisis interventions as needed. Training and Professional Development Facilitate staff training sessions on relevant topics, including case management best practices, cultural competency, trauma-informed care, and crisis intervention. Support staff in acquiring the skills and knowledge needed to provide high-quality care to youth. Collaboration and Networking Establish and maintain collaborative relationships with community partners, service providers, and government agencies to enhance resources and support for clients. Represent The Ali Forney Center at meetings, conferences, and events to advocate for the needs of LGBTQIA+ youth experiencing homelessness. QUALIFICATIONS Undergraduate Degree required. Minimum of two years of experience in case management. Knowledgeable in the skills of therapeutic engagement and basic assessment. Knowledge of LGBTQ+ issues, youth homelessness, trauma-informed care, and relevant social service systems. Experience working with the adolescent/young adult population required. Comfortability and competency working with transgender individuals. Effective computer skills and proficiency in Microsoft Office and Google Suite. Demonstrated ability to lead and mentor a team effectively. Strong organizational skills with attention to detail, ability to manage multiple tasks simultaneously. Ability to work collaboratively in a diverse team environment and build rapport with clients from diverse backgrounds. Computer knowledge and competent writing skills. Strong written and oral communication. Basic assessment and counseling skills. PREFERRED QUALIFICATIONS Bachelor's or Master's degree in Social Work, Human Services, Psychology, Public Administration, Nonprofit Management, or other discipline. Bilingual (Spanish or other commonly spoken community languages) Knowledge of local resources, social service systems, and advocacy frameworks related to homelessness and LGBTQIA+ rights in New York City. Familiarity with utilizing harm-reduction approach, sex-positive approach Ability to work independently and with others. Ability to use resources to problem solve. Trans and BIOPC individuals are encouraged to apply. Demonstrated knowledge of navigating NYC social service agencies, including the Runaway and Homeless Youth (RHY) system, shelter programs, and transitional housing resources. WORK ENVIRONMENT & PHYSICAL DEMANDS Regular and consistent interaction with clients, Youth Counselors, and other program staff. Exposure to individuals experiencing crisis or behavioral health challenges. Requires prolonged periods of sitting, standing, walking and moving throughout the building environment. Requires the ability to use a computer, telephone, mobile device and other standard office equipment for extended periods. Evening, weekend, holiday or schedule flexibility may be required based on program needs.
