Supervisor – Full Time – Days – Wyandotte Full Time Benefits Eligible General Summary The Supervisor of Integrated Case Management for Population Health is responsible for overseeing daily case management operations under the guidance of the Manager or Director of Integrated Case Management. This entails monitoring and guiding a team of professionals responsible for the collaborative practice of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health care needs through communication and available resources to promote patient safety, quality of care and cost‑effective outcomes. The Supervisor is responsible for participating in the development of policies, procedures, daily workflow design, and the maintenance of performance improvement activities. Education/Experience Required Bachelor's degree in nursing or a related professional field (e.g., social work, counseling, health education) or a Master's Degree in Social Work Three (3) years of clinical experience in case management, disease management, health coaching, nursing, or home health care Strong communication and interpersonal skills Strong analytical, problem‑solving, and critical thinking skills Ability to develop collaborative relationships within an interdisciplinary care team Certifications/Licenses Required Registered Nurse (RN) or a Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license Preferred Certification in Case Management (CCM) from the Commission for Case Management Certification (CCMC) Additional Information Organization: Henry Ford Wyandotte Hospital Department: Case Management WHMC Shift: Day Job Union Code: Not Applicable #J-18808-Ljbffr Henry Ford Health
