Registered Nurse Home Health Case Manager

Job Description The Registered Nurse Home Health Case Manager plays a vital role in delivering comprehensive nursing care to patients in their home environments. This position involves assessing patient needs, managing medications, educating patients and caregivers, and coordinating multidisciplinary care within a team-based caseload. The nurse will utilize an electronic health record system to document care and communicate effectively with all stakeholders to ensure optimal patient outcomes.
