Center Medical Director

Title Center Medical Director Company Oak Street Health Role Description The purpose of the Center Medical Director role at Oak Street Health is to lead the clinical team at their center in providing the best patient care and a great place to work in order to keep patients happy, healthy and out of the hospital. The Medical Director will practice primary care in our clinics alongside their provider colleagues at 80% clinical time and 20% administrative time. The CMD will have mission‑critical leadership responsibilities, which include setting an example for how thoughtful primary care is practiced, serving as a formal mentor, coach, and advisor to providers in Oak Street clinics, and participating in critical organization‑wide strategic initiatives. Core Responsibilities Support of Organizational Strategy Work in tight collaboration with the Practice Manager and Outreach Manager/Director to achieve our organizational objectives: Best Care Anywhere, Unmatched Patient Experience, Exceptional Growth, Great Place to Work and Leader in DE&I. Drive implementation of OSH strategic initiatives. Follow Direction from Regional Leadership Implement regional priorities and reach regional objectives in collaboration with the Regional Medical Director or Senior Medical Director. Cascade organizational information to care team members and elevate concerns and ideas to the rest of the organization. Provider Management Coordinate and lead the onboarding of new providers at the center; model excellent medical care, implement the care model and support metrics. Ensure care teams prepare for and execute daily huddles, care‑planning, admission prevention, and documentation workflows. Maintain evidence‑based care and implement Oak Street’s clinical guidelines at both individual and center level. Provide ongoing support, evaluate metrics, and address root causes when center metric goals are not met. Leadership and Coaching Create and maintain a culture of coaching and growth for providers and care teams. Conduct performance reviews, discuss provider goals, and develop plans to achieve them. Address concerns with constructive problem‑solving. Culture Acknowledge and celebrate successes of care team members, communicate opportunities for improvement, and support a positive, joyful practice environment. Achieve triad best practices. Other duties as assigned. Qualifications MD, DO, A.P.N., or P.A. graduates. Board certification in Internal Medicine or Family Medicine (MD/DO) or Family, Adult, or Gerontology (A.P.N.). Experience in clinical leadership roles, leading and coaching providers. Experience using a metrics‑driven approach to medical care and/or quality projects. Passionate about work and motivated to be part of a fast‑growing organization. Commitment to older adults and low‑income patients. Comfort with ambiguity and a love for problem solving. Ability to remain calm in the midst of the storm, guiding others to smart, effective solutions. 5+ years of experience in outpatient practice (preferred). Fellowship training in Geriatrics and other professional degrees (e.g., MBA, JD, MPH) welcomed but not required. Prior executive roles welcomed but not required. US work authorization. Work Hours 40 hours per week, full time. Pay & Benefits Pay range: $174,070.00 – $374,920.00 per year (base salary). The base salary will depend on experience, education, geography, and other factors. The position is eligible for a comprehensive benefits package that includes medical, dental, and vision coverage; paid time off; retirement savings options; wellness programs; and other resources based on eligibility.
