Licensed Practical Nurse (LPN) – Home Health

Schedule: Monday–Friday, 8:00 AM–5:00 PM
Guaranteed Hours: 40 hours per week
On-Call: 1 weekend every 4 weeks
License Required: Georgia LPN License (Compact License accepted)
Productivity Expectation: 30 points per week
Mileage Reimbursement: $0.43 per mile
Territory: Bartow and Paulding Counties, GA

Position Overview

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide high-quality home health care to patients in Bartow and Paulding Counties. The ideal candidate is committed to delivering patient-centered care while promoting independence, dignity, and positive health outcomes in the home setting.

Required Qualifications

• Active Georgia LPN license or Compact LPN license
• Home Health experience
• Proficiency with HomeCare HomeBase (HCHB)
• Previous OASIS documentation experience
• Strong clinical assessment and documentation skills
• Valid driver's license, reliable transportation, and auto insurance

Responsibilities

• Deliver skilled nursing care according to individualized physician orders and the patient's plan of care.
• Monitor and assess patient conditions, including vital signs, and report significant changes to the care team.
• Administer medications, treatments, oxygen therapy, ostomy care, and other skilled nursing interventions.
• Collaborate with physicians and interdisciplinary team members, including Physical Therapy, Occupational Therapy, and Speech Therapy, to coordinate patient care.
• Complete accurate and timely OASIS assessments and clinical documentation in HomeCare HomeBase (HCHB).
• Maintain infection prevention and control standards while identifying and reporting potential communicable diseases or infections.
• Monitor patient progress toward established goals and communicate updates to the appropriate healthcare providers.
• Order and maintain necessary medical supplies, medications, and nutritional support to ensure continuity of care.
• Respond appropriately to patient incidents, complete required documentation, and notify appropriate personnel.
• Inspect and monitor medical equipment, coordinating repairs with durable medical equipment (DME) providers as needed.
• Educate patients and caregivers on disease management, medications, safety, and home health best practices.
