MDS Coordinator
Join our 5-Star Post Acute/Long-term Care Facility
Inglemoor Rehabilitation and Care Center, a leader in the skilled nursing industry, is seeking a dedicated and experienced MDS Registered Nurse to join our healthcare team on a Full Time basis in Livingston, New Jersey .
About the Role
As a key member of our interdisciplinary team, the MDS Coordinator will play a crucial role in ensuring the highest level of care for our residents. The ideal candidate will have at least 3 years of experience in MDS and long-term care regulations, with a strong background in healthcare and a passion for delivering exceptional patient care.
Responsibilities
Coordinate and maintain the Minimum Data Set (MDS) process for all long-term residents, ensuring accurate and timely assessments
Develop and implement care plans that meet a resident-centered plan of care, in collaboration with the interdisciplinary team
Conduct MDS assessments, evaluations, and progress notes, ensuring compliance with regulatory requirements
Provide education and training to staff on MDS process and documentation requirements
Utilize EMR and HER systems effectively for documentation and reporting purposes
Familiar with EPIC, PCC, and EMR/EHR, CMS 5-star
Strong analytical skills with attention to detail for documentation review and coding accuracy
Ability to work independently as well as part of IDCP team
