Upperline Health providers coordinate patients’ care among a team of specialists – Podiatric Physicians and Surgeons, Primary Care Physicians, Endocrinologists, Advanced Practice Providers, Care Navigators, Pharmacists, Dieticians, and Social Workers for integrated treatment that addresses patients’ immediate and long-term health needs.
