Vertus HealthCare, a Medicare Certified home health agency, is seeking a dedicated and compassionate Registered Nurse to join our Per Diem team in Boynton Beach, Florida. As a Registered Nurse with Vertus HealthCare, you will have the opportunity to make a meaningful difference in the lives of our patients, providing high-quality, value-based healthcare in the comfort and privacy of their own homes.
About the Role:
As a Registered Nurse with our Per Diem team, you will be responsible for:
- Providing skilled nursing care to patients in their homes
- Assessing patient needs, developing, and implementing individualized care plans
- Collaborating with other healthcare professionals to ensure comprehensive care
- Educating patients and their families on disease management, medication instructions, and self-care techniques
- Monitoring patient progress, reporting changes, and making adjustments to care plans as necessary
Responsibilities:
- Conduct patient assessments, develop, and implement individualized care plans
- Provide skilled nursing care, including wound care, medication administration, and infection control
- Collaborate with other healthcare professionals, including physicians, therapists, and home health aides
- Educate patients and their families on disease management, medication instructions, and self-care techniques
- Monitor patient progress, report changes, and make adjustments to care plans as necessary
- Maintain accurate and detailed records of patient care and treatment

Requirements:
- Current RN license in the state of Florida
- Minimum of 1 years of experience as a Registered Nurse
- Current CPR certification
- Ability to work effectively in a fast-paced environment
- Strong communication and interpersonal skills
