CNA - Home Health (Full Time)

CNA - Home Health & Hospice Shift Available: Full Time (Monday-Friday) Service Area with Openings: Northwest Responsibilities: Provide exceptional care in coordination with the assigned nursing care team. Maintain patient records that include any changes in doctor’s orders, diet, or personal living activities. Help with patient meals, feeding, and basic activities of daily living. Deliver weekly reports to the case manager and review any important changes in patient conditions. Educate patients and family members. Prepare timely, complete, and accurate clinical notes. Communicate effectively with healthcare providers and other care team members. Qualifications: CNA license in the state of Texas (required). One (1) year home health and hospice experience . Able to lift up to fifty (50) pounds. Able to work occasional weekends, holidays, evenings. Strong communication and people skills.
