The PT will collaborate with patients, families, caregivers, and referring providers while maintaining accurate clinical documentation.\n Responsibilities:\n \n Perform comprehensive patient examinations and screenings to determine appropriate therapy services.\n Evaluate clinical findings and establish physical therapy diagnoses and outcomes of care.\n Develop individualized plans of care incorporating goals, treatment, outcomes, and discharge planning.\n Evaluate and treat impairments related to orthopedic and neurologic conditions.\n Perform objective testing including range of motion, strength, and functional performance.\n Safely perform and assist with patient transfers between bed, chair, and floor as appropriate.\n Demonstrate proper body mechanics and safe patient-handling techniques.\n Modify treatment plans based on patient response and current evidence.\n Provide patient, family, caregiver, and healthcare-provider education.\n Apply therapeutic taping for stabilization, facilitation, or inhibition of movement.\n Perform appropriate therapeutic modalities, including ultrasound, interferential, and iontophoresis.\n Communicate patient progress and treatment recommendations with referring providers.\n Monitor and provide feedback to support personnel as applicable.\n Complete accurate clinical documentation and daily charging in the therapy EMR.\n Follow applicable clinical, legal, ethical, and organizational standards.\n Required Qualifications:\n \n Bachelor’s degree in physical therapy\n Active Illinois Physical Therapist License (IDFPR)\n BLS – American Heart Association\n Experience with patient transfers, patient examination, and patient screening\n Knowledge of physical therapy diagnoses and treatment planning\n Experience with orthopedic and neurologic conditions\n Proficiency with ROM, strength, and functional testing\n Experience with clinical documentation and therapy EMR\n Additional Skills:\n \n Therapeutic Modalities\n Ultrasound\n Interferential\n Iontophoresis\n Patient Education\n Communication with Referring Providers\n Clinical Documentation\n Patient Safety & Body Mechanics\n Schedule:\n Monday–Friday | 8:00 AM–4:30 PM | 40 Hours/WeekNo weekends or holidays\n About Talent4Health\n \n A leader and innovator, offering comprehensive talent solutions for the healthcare industry across the nation.
