SUMMARY: The Clinical Diabetes Nurse will be an integral part of the team, behind the scenes and in the clinic. The Clinical Diabetes Nurse will work collaboratively with the providers and clinical staff to ensure high quality, timely care be provided. This is demonstrated using the nursing process to work collaboratively with the healthcare team in planning, directing, providing and evaluating patient care. The Clinical Diabetes Nurse is defined within the department to provide patient care, triage, consultation and referral among team members. ESSENTIAL DUTIES and/or RESPONSIBILITIES include the following: Greeting visitors and clients in a friendly and professional manner Triage and Development of Plan of Care and determining needs of the patients. Care coordination and pre-visiting planning Collaborates with an interdisciplinary team to evaluate clinical care or health services. Conducts monthly multidisciplinary team meetings with the approach to Diabetes. Medical records documentation with organized and quality documentation as required. Work collaboratively with providers to deliver high quality care. Work with the Endocrine Specialist with designated clinical times set aside for "Endocrine Clinic". Reviews and triages incoming test results as indicated comparing the data with the norm, when possible, to determine patient care needs and notifies the provider and assists with interventions appropriately and documents accordingly. Administers medications that require patient assessments. Arranges patient transfers and assesses the safety of moving patients. Patient education of disease process, prevention and insulin/insulin pumps including CGM care. This may include home visits with diabetic patient as needed. Attend online courses for improving Diabetes understanding with a goal of achieving certification in Diabetes Care and Education Specialist within 5 years of employment. Become cross-trained in the clinic to support workflow as required and accepts assignments in other areas and performs duties within individualized competency. Responsible for completing the Annual IHS Diabetes Audit. Attend online training for the Annual Audit orientation. Attend additional training as identified as needed. Community job sites, home visits, and hospital visits. Others duties as assigned. QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each duty satisfactorily. EDUCATION and/or EXPERIENCE: Previous experience in a patient care setting preferred Previous Endocrinology experience preferred but not required CERTIFICATES, LICENSES, and/or REGISTRATIONS: Current nursing tristate licensure is required. Current Basic Life Support (BLS) certification. Licensed Practical Nurse (LPN) required, Registered Nurse (RN) preferred. Must have valid Driver's License and be insurable. KNOWLEDGE, SKILLS, and/or ABILITIES: Knowledge of diabetic education services. Strong organizational and interpersonal skills. Ability to determine appropriate course of action in more complex situations. Ability to work independently, exercise creativity, be attentive to detail, and maintain a positive attitude. Ability to manage multiple and simultaneous responsibility and to prioritize scheduling of work. Ability to maintain confidentiality of all medical, financial, and legal information. Ability to complete work assignments accurately and in a timely manner. Ability to communicate effectively, both orally and in writing. Ability to handle difficult situations involving patients, providers, or others in a professional manner Excellent verbal and written communication skills Ability to effectively present information and respond to questions from providers, patients and their family members or other employees within the work setting PLEASE NOTE: P.L. 93-638, Indian Preference and Tribal Employment rights Ordinance.
