Location: Moraga Post Acute Pay Rate: $45.00–$50.00 per hour DOE Schedule: Full-Time | PM Shift | 4/2 Schedule

Moraga Post Acute is seeking a compassionate, dependable, and clinically skilled Registered Nurse (RN) to join our dedicated nursing team on a full-time PM shift with a 4/2 schedule. This is an excellent opportunity to become part of a collaborative team that is committed to delivering outstanding care in a supportive post-acute setting.

As an RN, you will play a vital role in overseeing resident care, leading the nursing team during your shift, and ensuring our residents receive the highest quality clinical services while maintaining their dignity, comfort, and well-being.

Key Responsibilities

• Provide skilled nursing care in accordance with individualized care plans
• Administer medications, treatments, and physician orders accurately and safely
• Assess residents' conditions and promptly identify and report changes
• Supervise and support LVNs and Certified Nursing Assistants (CNAs)
• Maintain accurate and timely clinical documentation
• Collaborate with physicians, therapists, and the interdisciplinary care team
• Assist with admissions, discharges, assessments, and care planning
• Ensure compliance with all federal, state, and facility regulations
• Promote infection prevention, resident safety, and quality care initiatives
• Foster a positive, respectful, and resident-centered environment

Qualifications

• Current, active California Registered Nurse (RN) license
• Strong clinical assessment and critical thinking skills
• Excellent communication and leadership abilities
• Ability to thrive in a fast-paced skilled nursing environment
• Previous experience in post-acute, skilled nursing, or long-term care preferred, but not required
• Passion for providing compassionate, high-quality resident care
