As a Registered Nurse, acts as a Charge Nurse. Incumbent will assist the Director of Nursing (DON)/Assistant Director of Nursing (ADON), as directed, in assessing, planning, providing, and documenting direct and indirect nursing care for skilled nursing facility residents in accordance with the physician's plan of care. Participates in the assessment of resident needs and the development and coordination of resident care plans. All nursing practice is based on the legal scope of practice, national and specialty nursing standards, BST policies and procedures, and in accordance with all applicable laws and regulations. The RN also provides nursing care interventions, preventing complications and promoting improvement in the residents' comfort and wellness, and informing the interdisciplinary team of their condition as appropriate. All efforts are directed towards continuous quality improvement and resident-centered care. Performs assigned managerial tasks with a customer service focus. Performs procedures to include the following in accordance with BST policies and generally accepted nursing practices: medication administration and recording; oxygen administration; insert and irrigate Foley catheters; tracheotomy care; perform Heimlich maneuver; provide decubitus assessment and care; perform gastrostomy tube procedures; perform cardiopulmonary resuscitation (CPR); utilize Automated External Defibrillator (AED). Work hours, especially when assigned on weekends, are essential to the Payroll Based Journal (PBJ) documentation for the Center for Medicaid and Medicare Services (CMS) must fully complete assigned shift when assigned weekends

What you will do in this role:

• Supervises and provides direct nursing care according to physician orders and in conformance with recognized nursing techniques and protocols, established standards, and administrative policies.
• Delegates and supervises and leads the care provided by certified nursing assistants (CNAs) and licensed vocational nurses (LVNs).
• Assures quality of care by adhering to therapeutic standards, measuring health outcomes against resident care goals and skilled nursing or regulatory standards.
• Performs admission, discharge, and transfer procedures, and assists with resident's activities and care.
• Executes physician's orders for all assigned residents; safely and properly administers medications and treatments according to State laws and regulations and standard nursing practices.
• Assesses residents; determines nursing diagnosis; plans, implements, and evaluates resident care upon admission and throughout stay. The assessment must include biophysical, psychosocial, environmental, self-care, education, and discharge planning factors incorporating data from the resident and or family when appropriate, in collaboration with the interdisciplinary team.
• Records all pertinent treatment information on assigned residents to include observations, treatments, and medications in Electronic Medical Record (EMR); keeps DON, ADON and physician informed of any adverse changes in condition of same.
• Verifies physician orders accurately and acknowledges new orders in the EMR. Maintains documentation that exemplifies adherence to the highest standards of practice.
• Provides accurate inventory of medication/nursing supplies by checking stock to determine inventory par levels.
• Makes periodic rounds of each assigned resident, record needs, conditions, and reports adverse situations, as applicable, to the DON, ADON and Physician.
• Provides oncoming Charge Nurse with resident reports and receives same from departing shift, as required.
• Performs routine nursing duties to include baths, oral hygiene, temperature, pulse, respiration, blood pressure, nutrition, And diet management. Record assessments, feeding, exercise, dressing changes, specimen collection, and postmortem care.
• Maintains a cooperative relationship among team members by communicating information and participating in team problem-solving methods.
• Promotes and maintains respect for rights, privacy, and confidentiality of others. Respects resident and staff confidentiality at all times.
• Performs other general tasks as required to provide for the safety and physical, spiritual, and emotional comfort of residents.
• Know and follow established emergency procedures; administers appropriate medical care, notifies appropriate officials, carries out physician orders and provides leadership and proper response.
• Maintains professional and technical knowledge by attending educational workshops, in-services, and participating in professional societies.
• Cooperates and is reliable with providing on-call coverage as scheduled and consistent with BST policies and procedures.
• Performs other duties as assigned by supervisor.

Qualifications

Certifications/Qualifications:

• Required: Must be a Registered Nurse licensed by, and in good standing with, the Texas Board of Nurse Examiners. At least one year of previous experience in a long-term care setting
• Required: Current RN license from the State of Texas and/or Compact RN license upon hire. Must have current CPR and AED certifications.
• Required: Must be current in attaining the Continuing Education Units(CEU's) necessary to meet licensing requirements and participate in continuing education programs, when directed.
• TRAINING: Required: Continued education according to regulations and Texas Board of Nursing.
• Preferred: Bachelor of Science in Nursing or some education beyond the minimum licensing level is desirable. Education in leadership and management is highly desirable.
• Preferred: Specialized training in nursing, supervisory skills, and infection control, is desirable.

WORK SCHEDULE: Work is normally scheduled to coincide with one of three standard nursing shifts to allow coverage for a 24-hour, 7 day-a-week operation. Employee will be expected to work on-call duties as assigned. Exact duty hours are established by the DON and the Administrator. Employee will be advised of the assigned schedule in advance.

WORK LOCATION: The Mission at Blue Skies of Texas - East.

ATTIRE AND DRESS CODE: Prescribed nursing uniforms or scrubs are appropriate for wear. Slip-resistant, closed-toe shoes required. Blue Skies employee ID badge or name tag must be worn at all times while on duty. Gowns, gloves, safety belts, and other items of personal protective equipment clothing are worn, as needed, and training will be provided to ensure safety and proper wear during use.
