The Sub-Acute and Long-Term Care sector is rapidly growing, offering excellent opportunities and a flexible work-life balance (no on-call duties or weekend shifts)! We are currently looking for a Full-Time provider (32-40 hours weekly) for our Lebanon Center situated in Lebanon, NH. This role is open to either a nurse practitioner or a physician assistant, and we encourage new graduates to apply.
The annual salary for this position ranges from $112,320 to $124,800, along with an incentive compensation bonus program that has no cap.

Key Responsibilities- The Advanced Practice Provider will report directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager.
- Collaboration with the Center Leadership Team will be essential to meet center-specific goals and priorities.
- Working alongside the Medical Director and/or Attending Physician, the Advanced Practice Provider will deliver patient-specific medical care.
- Primary and urgent care services will be provided to nursing center residents in partnership with other medical providers at the center.
- Collaboration with Center Leadership is crucial to achieving our mission of enhancing lives through high-quality healthcare and compassionate service.
- The Advanced Practice Provider must have graduated from an accredited School of Nursing, hold a registered nurse license, and be certified as a Nurse Practitioner with a Master's Degree in Nursing, focusing on Older Adult and/or Family Nurse Practitioner (FNP). Other specialties may be considered based on experience and organizational needs.
- For Physician Assistants, graduation from an ARC-PA accredited program is required, along with passing the PA National Certifying Exam (PANCE) to obtain the PAC designation. State licensure and NCCPA certification are mandatory.
- A minimum of two years of experience working with adult or geriatric populations is preferred.
- In states where practice regulations permit, an APRN must hold an unrestricted professional license and DEA for each state of practice. Similarly, PAs must have an unrestricted professional license and DEA for their respective states.

Note: Restrictions may apply based on collective bargaining agreements, state laws, and factors such as pay classification, job grade, location, and length of service. To qualify for full-time benefits, a minimum of 30 hours per week is required. Certain benefits, including health plans, may not be available to part-time employees working between 16 and 29 hours per week.
Salary InformationPosted Salary Range: $112,320.00 - $124,800.00 per year. Bonus: $5,000.00.
