LTSS Service Care Manager
Immediate need for a talented LTSS Service Care Manager. This is a 12+ Months Contract opportunity with long-term potential and is located in New Port Richey, FL, Port Richey, FL, Hudson, FL, or Land O' Lakes, FL (Hybrid). Job ID:26-32446 Pay Range: $27 - $29/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- Manage a caseload for healthcare members with long-term care needs.
- Conduct monthly and quarterly member contacts, with approximately 80% travel.
- Perform assessments with members, caregivers, or providers to gather essential information for care plan development.
- Monitor service delivery and follow up with members and caregivers through in-person visits and telephonic contact.
- Authorize and coordinate referrals for services, ensuring no gaps in provider services.
- Collaborate with discharge planners, physicians, and other stakeholders to create effective discharge and care plans.
- Assist members in filing and resolving complaints and appeals.
Key Requirements and Technology Experience:
- 2+ years of experience in care management, specifically in long-term care.
- Proficiency with electronic medical health records and Microsoft Office.
- A valid driver's license is required for travel.
- A Bachelor's degree in a relevant field.
- Experience in home visits and familiarity with care management tools.
