Job Summary:

We are seeking a compassionate and experienced Social Services Director to join our team. As a key member of our organization, you will be responsible for ensuring that our residents receive exceptional care and support while providing a welcoming and inviting environment for all. This is a full-time position with an annual salary range of $40,000 to $50,000 based on years of experience in social services roles, including length of time in long-term care.

About the Role:

As Social Services Director, you will be responsible for:

• Overseeing the daily operations of social services, including discharge planning, home health services, and patient advocacy

• Ensuring compliance with federal and state regulations, including pasa regulation rules

• Developing and implementing policies and procedures to promote efficient and effective social services delivery

• Collaborating with other departments to ensure seamless transitions for residents

• Providing timely follow-up on grievance procedures and patient concerns

• Fostering a positive and supportive environment that promotes resident satisfaction and well-being

Requirements:

• 2 years of previous experience in a social services role, preferably in long-term care

• Strong knowledge of discharge planning, home health services, and patient advocacy

• Ability to ensure compliance with regulations and policies

• Excellent communication and problem-solving skills

• Ability to work effectively in a fast-paced environment
