Nurse Practitioner / Physician Assistant

Physical Medicine and Rehabilitation (PM&R) or Physiatry is a branch of medicine that aims to enhance and restore functional ability and quality of life to those with physical impairments and disabilities, with a focus typically on the neuromusculoskeletal system. As an NP/PA PM&R Clinician with Comprehensive Rehab Consultants (CRC), you will work closely with the post-acute therapy team to optimize rehabilitative outcomes and promote patient independence. This position involves treating patients exclusively at a designated skilled nursing facility (SNF) or post-acute rehab center. Position Information Location: Fresno, CA Job type: Full-time, 24-hour, W-2 role Schedule: 8‑hour shifts with flexibility, typically 9AM – 4PM; 3-day work week with days selected during onboarding Hours: 9AM – 4PM (flexible) Compensation: $75,000 – $95,000 plus bonus pay Relocation: Requires reliable commute to Fresno or intent to relocate before start Benefits Health insurance (Medical, Vision, Dental) Short‑Term Disability, Life Insurance, Critical Illness Generous PTO package Flexible hours Highly competitive salary and generous bonus $600 continuous education reimbursement; opportunity to earn a minimum of 33 CEUs at no cost Full‑time Virtual Scribe ($1,368/month value) Access to CRC’s Physiatry certification and CEU credits 401(k) plan with annual contribution of 2‑3% 100% malpractice insurance coverage Company laptop Responsibilities Provide specialized PM&R/Physiatry services as a rehab consultant Represent CRC in the facility through strong clinical leadership and collaboration with the interdisciplinary care team Collaborate with PCP teams to manage conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries Work alongside PT, OT, and SLP teams to improve patient function and quality of life Follow therapy patients to track progress in therapy Assess and collaborate on management of barriers to therapy, including joint or neuropathic pain, spasticity, neurogenic bowel, or neurogenic bladder Attend multidisciplinary meetings and bridge the gap between therapy and internal medicine Engage in culture‑building opportunities and ongoing education, including monthly didactic lectures, virtual Regional Team meetings, and annual regional dinners Ideal Clinician Passion for post‑acute healthcare and serving the geriatric population Experience in SNF or post‑acute settings (preferred) Confidence to function autonomously Desire for continuous improvement and ability to embrace change and growth Strong interpersonal skills and understanding of the needs of facility partners and patients Empathy and compassion Willingness to invest in company culture Qualifications NP/APRN/Physician Assistant License California License Eligibility to work in the U.S. Experience in post‑acute space (preferred) Great attitude and desire to deliver the best patient care #J-18808-Ljbffr
