Responsibilities

• Assist the Social Services department with supporting residents and their families.

• Help residents adjust to the skilled nursing environment and address social and emotional needs.

• Assist with completing and maintaining resident documentation and records.

• Communicate with residents, families, staff, and outside resources as needed.

• Assist with care plan meetings and follow-up activities.

• Help coordinate appointments, referrals, and community resources.

• Assist with discharge planning and transition-related activities.

• Support resident rights, dignity, privacy, and quality of life.

• Participate in interdisciplinary team meetings and facility activities.

• Maintain confidentiality and follow all facility policies and applicable regulations.

Qualifications

• Previous experience in a healthcare, long-term care, social services, or customer-service environment preferred.

• Strong communication and interpersonal skills.

• Compassionate and patient approach when working with residents and families.

• Excellent organization and attention to detail.

• Ability to work effectively as part of an interdisciplinary team.

• Proficiency with basic computer applications and documentation.

• High school diploma or equivalent required; additional education in social services, human services, psychology, or a related field preferred.
